1. Write a $10 check as payment for the first evening class on Jan.7/09.
2. Write personal information on a 3x5" card:    Name

3. Send by post to: L.B. Lee Art Studio

4. List your preference ( post, email, etc...) for receiving back a confirmation
    number on the class roster.  25 seats are available.
